Pharmacist / Senior Pharmacist (Outpatient Pharmacy) #HJO
Job Description
- Provide pharmaceutical care including dispensing and counselling of medications
- Educate patients and caregivers on proper medication use and adherence
- Identify, resolve, and prevent medication-related issues
- Ensure accurate and timely dispensing of medications
- Support safe administration of medications and provide guidance to healthcare staff
- Maintain proper documentation and comply with pharmacy standards and protocols
- Participate in quality improvement initiatives and projects
- Collaborate with healthcare teams to ensure optimal patient care
- Perform other duties as assigned
Requirements
- Degree in Pharmacy or equivalent, recognised by the Singapore Pharmacy Board for registration
- Strong communication and patient counselling skills
- Good attention to detail and accuracy
