Job Summary and Responsibilities

As our Physical Therapist, you will help Hospice/Home Healthcare patients perform all phases of physical therapy evaluation, treatment planning, treatment, and documentation so they can achieve their rehabilitative goals and improve their quality of life in a home setting.

Every day you will evaluate patients, participate in treatment program planning, and administer physical therapy modalities, procedures, and tests.

To be successful in this role, you must have at least two years of professional experience in rehabilitation, excel at documenting patient progress, and effectively supervise Certified Home Health Aides.

  • Perform all phases of physical therapy evaluation, treatment planning, treatment, and documentation.
  • Evaluate patients, participate in treatment program planning, and administer physical therapy modalities, procedures, and tests.
  • Document accurate patient records and progress notes, maintaining appropriate oral or written contact with referring physicians and co-workers.
  • Ensure a clean and safe work environment by being alert for unsafe conditions and malfunctioning equipment and implementing corrective procedures.
  • Supervise Certified Home Health Aides in their participation in the overall treatment program.
  • Assist DPCS, Hospice Supervisor, and QI Coordinator with Quality Improvement (QI) activities as required.

Job Requirements

Required

  • Two (2) Years Of Professional Experience In Rehabilitation
  • Active Physical Therapist (PT) license in the State of California
  • Basic Life Support – Cardiopulmonary Resuscitation (BLS-CPR)
  • Driver's License: California
  • Auto Insurance
