Job Summary and Responsibilities

As our Physical Therapist, you will help patients rebuild or maintain their physical wellbeing so they can function at their best level, reduce pain, and improve their quality of life.Every day you will perform initial evaluations, develop and update individualized plans of care, and provide a variety of direct treatments. You will be expected to consistently meet patient care productivity standards, collaborate with the interdisciplinary team and families, and promote a positive customer service environment.To be successful in this role, you must possess strong age-appropriate competency skills, excellent communication abilities, and a collaborative, patient-centered approach to care.

  • Patient Care: Assists in the administration of treatment using accepted Physical Therapy procedures under the supervision of a licensed Physical Therapist. Teaches patient and/or caregiver in functional activities and treatment techniques of speech therapy as appropriate to meet their needs.
  • Documentation: Documents therapy on the visit record. Uses clear, concise and objective data. Records treatments accurately according to the treatment plan.
  • Communication Skills: Communicates patient progress toward goals and rehabilitative potential to other members of the healthcare team. Collaborates with other team members to insure appropriate utilization of services.
  • Standards of Conduct: Maintains the standards of conduct as promulgated by the American Physical Therapy Association (as a registered Physical Therapist Assistant); adheres to the professional code of ethics in the performance of duties.
  • Clinical Knowledge: Demonstrates specialized clinical knowledge and skills in an identified area of expertise.
  • Compliance with Policy and Procedures: Complies with safety, infection control, CHW Integrity Program, and other organizational policies. (90 day evals - department specific orientation checklists completed; annual evals - annual competency checklists completed) Complies with the organization's dress code. Arrives at work on time, as defined by the Timeliness and Attendance Policy. Attends work consistently, as defined by the Timeliness and Attendance Policy.

Job Requirements

Required

  • One (1) Year Acute Care or SNF experience
  • Physical Therapy Assistant: CA, upon hire and
  • Basic Life Support - CPR, within 30 Days
