Job Description and Performance Standards

Title: Certified Physical Therapist Assistant 554386401

Department: Hamilton Home Health

Reports To: Therapy Supervisor

JOB SUMMARY

Under the direction of a registered physical therapist, utilizes physical therapy skills to assist in treatment of referred patients of all age populations. Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served. Must demonstrate knowledge of the principles of growth and development and assess data reflective of the patients’ current status in relation to their age and medical needs.

JOB QUALIFICATIONS

Education: Graduate of an accredited program for Physical Therapy Assistant.

Licensure: Registered PTA, or eligible for license in State of Georgia. Must have a valid driver’s license and automobile insurance coverage in accordance with Hamilton Health Care System Driver Requirements policy.

Experience: One (1) year experience

Skills: Basic first aid and CPR techniques and license. Ability to obtain and interpret information in terms of the patients needs. Supplies skills appropriate for age group of patient being served.

PHYSICAL, MENTAL, ENVIRONMENTAL AND WORKING CONDITIONS

Works in the home setting. Required to display characteristics consistent with certification requirements and ethical practices of physical therapy. Must be able to meet state documentation requirements. Considerable amount of driving required. Ability to lift and transfer patients. Requires full range of motion, manual and finger dexterity and eye-hand coordination. Requires corrected vision and hearing to normal range. Requires some exposure to communicable diseases or bodily fluids.

1. Assists in performing treatment program for all age populations as developed and directed by the registered physical therapist. Demonstrate consistent ability to perform clinical skills necessary in the care and treatment of all ages but primarily those whom are geriatrics.

1.1 Acquires and maintains the skills necessary to perform treatment and procedures in Home Health as directed by RPT.

1.2 Prepares patients according to procedure through proper draping and positioning for procedure to be administered.

1.3 Administers patient treatment as directed by the RPT, in accordance with established procedure.

1.4 Monitors patient during treatment and modifies treatment in consultation with the RPT with no more than 2 documented patient incidents per year.

1.5 Documents ongoing treatment according to department procedure.

1.6 Provides for patient safety at all times, as per procedure.

1.7 Assists in the safe patient transport as requested by RPT.

1.8 Assists with instructions of the family as assigned, documenting treatment and response accordingly.

2. Maintains and completes documentation department records and reports.

2.1 Maintains and completes accurately written patient care records in a timely manner in compliance with JCAHO and department procedures.

2.2 Records all patient charges accurately before the end of each day.

2.3 Completes any special documentation established for quality improvement purposes within requested time frames.

2.4 Assists in preparation of daily schedule, as demonstrated by less than two scheduling conflicts every six months.

3. Contributes to the efficient productive operation of the Home Health Department.

3.1 Demonstrates effective time management skills by completing patient care assignments within require time frame.

3.2 Demonstrates ability to organize patient care and see appropriate priorities by effective scheduling.

3.3 Demonstrates teamwork as indicated by assisting other staff members.

3.4 Manages use of equipment and treatment space efficiently by coordinating with other staff members.

3.5 Maintains continuous effective communication among the home health staff by participating in interpersonal communication to ensure the continuity of patient care.

3.6 Orients new personnel as requested according to department personnel.

3.7 Completes other duties as assigned.

4. Maintains commitment to professional growth and competency.

4.1 Identifies own learning needs by seeking colleagues input on unfamiliar or unusual procedures or cases.

4.2 Maintains professional skill level as documented by attendance at professional seminars and departmental in-services, as assigned by supervisor and required by law and regulation.

4.3 Reviews current literature in the field as demonstrated by contributing this to staff in-services or conferences.

4.4 Assists with at least one (1) in-service per year to Home Health Staff.

4.5 Assists with interdepartmental in-services as requested.

4.6 Instructs students and volunteers as appropriate to their educational level according to established Departmental procedure.

4.7 Participates in community education program as requested.

4.8 Completes special projects/program development as deemed appropriate by Director by participating in at least 2 of the following annually:

a. formal continuing education

b. community education programs

c. new program development

d. health support groups

e. policy/SOP development

f. Quality Improvement

g. student supervision

h. staff education

i. in-service

5. Operates and maintains equipment in a safe and orderly manner.

5.1 Acquires and maintains skills necessary to use equipment properly according to department procedure.

5.2 Notifies supervisor of any equipment failure or problems as soon as identified.

5.3 Utilizes equipment according to safety standards for each equipment type and according to established procedure.

5.4 Utilizes good body mechanics in the operations of equipment as demonstrated by no reported incidents.

5.5 Returns equipment to proper storage area in good working order with no more than 1 valid complaint by coworker.

5.6 Keeps equipment and treatment areas clean.

5.7 Monitors supplies and reports any need for reordering to designated person.

6. Promotes the Hamilton Medical Center philosophy to ensure quality of care.

6.1 Maintains client privacy and confidentiality at all times according to established procedures.

6.2 Provides positive, supportive communication to patients, families, visitors, and medical center personnel.

6.3 Collaborates with other departments of the medical center and outside agencies to meet the identified needs of the patient.

6.4 Clarifies directions or communications, which are unclear as demonstrated by correct follow through at all times.

6.5 Assist Medical Center visitors with information or directions whenever requested or the visitor appears in need of help.

6.6 Introduces self and position to patient, families, and visitors at all times.

6.7 Puts patient and/or family at ease by explaining procedure prior to initiating any procedure.

6.8 Assesses home environment for safety hazards, which could harm patients, visitors, or other hospital employees, and reports hazards to appropriate supervisors.

6.9 Ensures that the philosophy of Hamilton Medical Center is followed by employees to provide quality patient care..

6.10 Reports to work promptly as scheduled with less than 12 incidents of tardiness per review year.

6.11 Promotes teamwork and an efficient and effective professional working environment.

6.12 Dresses appropriate to specific job in a manner that reflects the professional image of Hamilton including proper wearing of identification badge.
