The Medical Director for the Home-Based Primary Care Program (HBPC) is a Physician with skills and experience in providing Primary Care to Veterans in their home and special interest in Geriatrics and Extended Care, who is responsible for the overall medical care delivered by the HBPC team. This position is approximately 40% administrative and 60% clinical. The clinical work will be divided between in-home care for Veterans enrolled in the HBPC program and support to the Long Term Care Unit coverage. This position will have an assigned panel of patients/Veterans from within the Tomah HBPC program. In addition to providing primary health care, the physician serves as a consultant to nursing, and medical staff, and interdisciplinary team members in providing quality care. The physician serves on medical center, VISN and/or national committees as appropriate, and assists in the development, evaluation, and communication of policies and standards to all healthcare disciplines.
