The Birmingham VA Health Care is recruiting for a Physician - Patient Aligned Care Team (PACT) Primary Care Provider for Women's Health service. VA offers a comprehensive total rewards package. Education Debt Reduction Program (Student Loan Repayment): This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Maintains an active role in the assigned multidisciplinary health care team, promotes preventive health measures, and manages the acute and chronic conditions of assigned patients. Provides comprehensive assessment, diagnosis work-up, and clinic intervention. This includes performing comprehensive patient assessments, incorporating patient health history, performing physical examinations, and conducting patient education. Such care includes both scheduled and unscheduled appointments. Acts as the clinical lead of the assigned PACT team to coordinate and follow-up on patient care. Includes daily huddling, scrubbing of schedules, pre-visit assessments, review of tests, patient notification of test results, and development of follow-up care plans. Work Schedule: Monday - Friday, 7:30 a.m. - 4:00 p.m. (subject to change based on the needs of the organization)
