Job Summary
The Physiotherapist is responsible for assessing, planning, and delivering evidence-based physiotherapy treatments to support patients in restoring movement, improving physical function, reducing pain, and enhancing overall quality of life. The role involves developing individualized rehabilitation plans, monitoring patient progress, educating patients and caregivers, and collaborating with multidisciplinary healthcare teams to achieve optimal patient outcomes while ensuring compliance with clinical and healthcare standards.
Job Description:
- Plan and run physiotherapy rehab services in the TCF [in coordination with the Rehab Manager and Occupational Therapist].
- Conduct assessments for patients during admission and periodic reviews.
- Institute an appropriate and effective treatment.
- Participate in multidisciplinary case discussions with the TCF care team.
- Contribute to formulation and follow-up of patients’ individual care plan [ICP] to ensure holistic health and care coverage.
- Plan and conduct group therapy programs and activities for patients.
- Take care of proper documentation of therapy records.
- Understand dementia issues and know how to manage patients who display behavioural problems [associate with onset of dementia]
- Take care of other duties which the superior may assign from time to time.
Job Requirements and Qualifications:
- Bachelor degree or Diploma in Physiotherapy.
- Qualified and registered with AHPC
- Minimum 3 years of working experience in a hospital or eldercare institution; preference is for a candidate with working experience in a community setting.
- Possess a passionate disposition to interact and care for seniors.
