About the Role
As a Partner in Wellbeing, the Psychiatric LPN handles pre-admission screening and assesses the mental health needs of clients under their care through in-person and virtual delivery methods. They work with other staff to develop a plan of care and assist clients and families with behaviors related to mental disorders. Take temperatures, record vital signs, collect urine samples, prep clients, etc. Assist with medication administration as prescribed by medical provider.
Essential Responsibilities
• Participates in the overall coordination and implementation of nursing support services for the medication management function of the organization.
• Assist in or ensure necessary medical-related client intake documentation is completed, reviewed, and issues addressed.
• Conduct a necessary review of systems such as heart rate, respiration, blood pressure, height, weight, or other applicable systems recommended by the medication management provider.
• Administering injections as prescribed by medical management providers.
• Collects necessary information to ensure accurate medication lists are documented in the client record.
• Provide customer service by promptly responding to client and community partner requests, phone calls, and/or other communications.
• Coordinate medication refills as needed or with other nursing staff.
• Provide needed medication education to eligible clients.
• Triage client requests and needs to ensure applicable interventions are provided timely.
• Work closely with medical records to ensure needed information is in the client record for coordination of care, such as lab reports, hospital discharge summaries, or others.
• Ability to work as a team and float between different psychiatric prescribers.
• Participates in all care coordination activities concerning clients on caseload as indicated by the Care Coordination managing the care-coordination function.
• Focuses on integrated health (physical, mental, substance use, and oral) care, ensuring that clients are referred to any necessary internal and/or external services to support their overall wellbeing.
• Ensures care coordination activities are client-centered.
• Performs additional duties as assigned or requested.
