Requirements<\/h3>
Perform <\/span><\/span>comprehensive psychiatric evaluations<\/span><\/span><\/b> for residents in <\/span><\/span>long\-term care and skilled nursing facilities<\/span><\/span><\/b>
<\/span><\/span><\/p><\/li>
Provide ongoing <\/span><\/span>medication management<\/span><\/span><\/b> and psychiatric follow\-up visits
<\/span><\/span><\/p><\/li>
Diagnose and treat common <\/span><\/span>geriatric psychiatry<\/span><\/span><\/b> conditions including <\/span><\/span>depression, anxiety, dementia\-related behaviors, psychosis, mood disorders, and chronic mental illness<\/span><\/span><\/b>
<\/span><\/span><\/p><\/li>
Prescribe, monitor, and adjust <\/span><\/span>psychotropic medications<\/span><\/span><\/b> based on each resident\u2019s clinical presentation
<\/span><\/span><\/p><\/li>
Develop individualized treatment plans that align with residents\u2019 psychiatric, medical, and psychosocial needs
<\/span><\/span><\/p><\/li>
Collaborate with <\/span><\/span>nursing staff, physicians, medical directors, social workers, therapists, and interdisciplinary care teams<\/span><\/span><\/b>
<\/span><\/span><\/p><\/li>
Provide behavioral health recommendations to facility staff to support continuity of care
<\/span><\/span><\/p><\/li>
Maintain timely, accurate, and compliant documentation within the medical record system
<\/span><\/span><\/p><\/li>
Communicate effectively with residents, families, guardians, and facility teams
<\/span><\/span><\/p><\/li>
Support quality psychiatric care delivery within the <\/span><\/span>nursing home / LTC setting
<\/span><\/span><\/b>

Qualifications:<\/span><\/span><\/b>
<\/span><\/span><\/p>
Board Certified <\/span><\/span>Psychiatric Mental Health Nurse Practitioner (PMHNP)<\/span><\/span><\/b>
<\/span><\/span><\/p><\/li>
Active and unrestricted <\/span><\/span>New York State Nurse Practitioner license<\/span><\/span><\/b>
<\/span><\/span><\/p><\/li>
DEA registration preferred or ability to obtain<\/span>
<\/span><\/span><\/p><\/li>
Strong clinical judgment and ability to practice independently<\/span>
<\/span><\/span><\/p><\/li>
Experience working with <\/span><\/span>geriatric, elderly, long\-term care, skilled nursing, nursing home, behavioral health, psychiatry, or post\-acute populations<\/span><\/span><\/b> preferred<\/span>
<\/span><\/span><\/p><\/li>
Prior experience managing <\/span><\/span>dementia, behavioral disturbances, chronic mental illness, or psychotropic medication regimens<\/span><\/span><\/b> strongly preferred<\/span>

Excellent communication and collaboration skills<\/span>

Must be comfortable working in <\/span><\/span>skilled nursing facilities / nursing homes / LTC environments<\/span><\/span><\/b>
