What You\u2019ll Do<\/b>

skilled nursing visits<\/b> in patient homes and care settings.
<\/div>\u2022 Deliver hands\-on care including
wound care, medication management, and assessments<\/b>.
<\/div>\u2022 Build consistent relationships with patients and families.
<\/div>
\u2022 Coordinate care with physicians, therapists, and interdisciplinary teams.

\u2022 Educate patients and caregivers on care plans and recovery goals.

What We\u2019re Looking For<\/b>

\u2022 Active Minnesota Registered Nurse (RN) license.

\u2022 Experience in
home health, med\-surg, rehab, or clinical nursing<\/b>.
<\/div>\u2022 Ability to work independently while staying connected to a team.

<\/div>
\u2022 Reliable transportation and willingness to travel locally.
