Responsibilities
- Manage and coordinate patient care from admission through discharge.
- Conduct comprehensive patient assessments and develop individualized plans of care in collaboration with physicians.
- Monitor patient progress and update care plans as needed to ensure optimal outcomes.
- Provide skilled nursing care, patient education, and disease management.
- Coordinate services with therapists, social workers, home health aides, and other healthcare professionals.
- Serve as the primary point of contact for patients and families, ensuring excellent communication and continuity of care.
- Advocate for patient needs and promote independence, safety, and quality of life.
- Complete accurate and timely clinical documentation in accordance with agency and regulatory standards.
- Participate in interdisciplinary team meetings and care coordination activities.
