Responsibilities
Patient Assessment & Care Planning
- Conduct comprehensive initial and ongoing patient assessments.
- Develop, implement, and update individualized care plans based on patient needs and physician orders.
- Identify therapeutic, preventive, and rehabilitative interventions to support optimal patient outcomes.
Direct Patient Care
- Provide skilled nursing services in accordance with physician orders and the State Nurse Practice Act.
- Administer medications, treatments, and clinical interventions as needed.
- Monitor patient progress and adjust care plans accordingly.
- Educate patients and caregivers on disease management, medications, and health maintenance.
Care Coordination
- Serve as the primary coordinator for all aspects of patient care.
- Collaborate with physicians, therapists, social workers, home health aides, and community providers to ensure seamless care delivery.
- Advocate for patients and help navigate resources that support independence and wellness.
Documentation & Communication
- Complete accurate and timely clinical documentation.
- Communicate changes in patient condition promptly to physicians and obtain necessary orders.
- Ensure compliance with all agency, state, and federal regulations.
Patient & Family Education
- Empower patients and families through education and support.
- Involve caregivers in the care planning process to promote confidence and successful outcomes.
Supervision & Leadership
- Provide direction, supervision, and support to home health aides.
- Conduct required supervisory visits and evaluations to ensure high-quality patient care.
- Serve as a clinical resource and leader within the care team.
