As a Hospice Registered Nurse (RN) – On Call, you serve as a trusted clinical presence during evenings, weekends, holidays, and urgent moments when patients and families need support most. This is a role for a nurse who thrives in an autonomous, field-based environment and is called to deliver compassionate guidance, rapid clinical decision-making, and exceptional end-of-life care outside of traditional business hours.

Role Overview

As a Hospice RN – On Call at VitalCaring, you are more than an after-hours nurse - you are the clinical decision-maker, care coordinator, and reassuring voice patients and families depend on outside of standard operating hours.

You’ll triage patient needs, provide telephonic support, complete urgent visits when clinically appropriate, and coordinate seamless care transitions across the hospice team.

Through each interaction, you’ll bring clinical expertise, responsiveness, empathy, and comfort to ensure patients receive timely, personalized care aligned with their goals and wishes.

What You’ll Do

Deliver Responsive After-Hours Hospice Care

• Respond to patient and caregiver calls with urgency, compassion, and clinical expertise

• Assess symptoms and patient needs telephonically and determine appropriate interventions

• Perform in-home visits as clinically indicated to support comfort and symptom management

• Deliver skilled nursing interventions focused on maintaining quality of life and reducing avoidable hospital utilization

Coordinate Urgent Patient Care

• Serve as the after-hours clinical liaison across patients, caregivers, branch teams, and providers

• Collaborate with physicians and interdisciplinary teams to address emergent care needs

• Monitor and respond to patient alerts and care requests through CareXM and internal systems

• Communicate timely handoffs and summaries of after-hours activity to branch leadership

Support Patients & Families

• Provide education, reassurance, and emotional support during vulnerable moments

• Help families navigate changing patient conditions with clarity and empathy

• Build trust through calm communication and responsive follow-up

• Ensure patients and caregivers feel supported, informed, and connected to their care team

Drive Quality & Documentation Excellence

• Maintain timely, accurate EMR documentation and same-day completion standards

• Ensure compliance with hospice regulations, company policies, and HIPAA requirements

• Participate in ongoing education and maintain knowledge of hospice best practices

• Utilize technology and communication platforms to support continuity of care

What You Bring

• Active RN license in state of practice (compact eligibility preferred where applicable)

• Graduate of an accredited nursing program

• Current CPR certification

• Minimum 1 year of clinical nursing experience required

• Strong clinical judgment and ability to independently manage urgent patient needs

• Excellent communication and de-escalation skills in emotionally sensitive situations

• Ability to work autonomously while maintaining close team coordination

• Basic EMR and Microsoft Office proficiency

• Valid driver’s license and reliable transportation

What Sets You Apart

• Hospice experience strongly preferred

• Minimum 3 years of hospice nursing experience preferred

• Experience with after-hours, triage, on-call, home health, or field-based care

• Strong symptom management and end-of-life clinical skills

• Experience coordinating care across interdisciplinary teams

• Proficiency with Homecare Homebase (HCHB)

• Ability to remain calm, organized, and compassionate in urgent situations
