What You\u2019ll Do<\/b>

skilled nursing visits<\/b> in patient homes and care settings.
<\/div>\u2022 Deliver treatments including
wound care, medication management, and assessments<\/b>.
<\/div>\u2022 Coordinate care with physicians, therapists, and interdisciplinary teams.

\u2022 Educate patients and families on
care plans, medications, and recovery goals<\/b>.
<\/div>\u2022 Maintain
accurate, timely documentation<\/b> in the EMR.

What We\u2019re Looking For<\/b>
<\/div>\u2022 Active Minnesota Registered Nurse (RN) license.
<\/div>
\u2022 Experience in
home health, med\-surg, TCU, rehab, or clinical nursing<\/b>.
<\/div>\u2022 Strong clinical judgment and ability to work independently.
<\/div>
\u2022 Excellent communication and patient education skills.
<\/div>
\u2022 Valid driver\u2019s license and reliable transportation.
<\/div>
