We provide skilled nursing care through Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) while our Certified Nursing Assistants (CNAs) and Medical Assistants (MAs) deliver essential support services. Our Certified Medication Technicians (CMTs) provide medication administration. To uphold the highest standards of care, all of our staff are thoroughly vetted through reference checks, background screenings, and drug testing.

The Registered Nurse is responsible for directly providing quality patient care. To access, plan, implement and evaluate the nursing process through LHS policies, procedures and standards, care will be provided within the frame work of the nursing services philosophy.

Ø Performs the initial nursing assessment of clients for admission criteria.
Ø Assists the physician in developing the initial and ongoing plan of care.
Ø Reviews and updates plan of care as necessary.
Ø Orients staff members to each case as applicable.
Ø Point of contact with the physician, pharmacist and referral sources regarding client’s condition and progress.
Ø Provides ongoing clinical monitoring.
Ø Monitors client’s inventory of pharmaceutical products and supplies as necessary.
Ø Ensures coordination of client services.
Ø Counsels and instructs the client/family in the set up and administration of infusion therapies.
Ø Attends case conferences and in service education programs/classes.
Ø Prepares clinical notes and enters them into the clinical record on individual client daily, and when necessary.
Ø Ensures clinical documentation meets LHS documentation standards.
Ø Administers medications and treatments as prescribed by the physician and the state’s practice act for Registered Nurses.
Ø Reports to Director of Nursing and/or the Physician any changes in the patient condition and needs.
Ø Adheres to administrative and patient care policies including the implementation of client rights and responsibilities.

1. Current license as a registered nurse in the state in which practicing.
2. One year experience as a registered nurse with population being served preferred.
3. Current CPR.

4. Physically able to sit, stand and walk a long distance if necessary.
5. Ability to drive long period of time.
6. Ability to bend, lift and carry medical supplies and equipment from one location to another without medical restriction.
7. Possess visual acuity and ability to differentiate colors.
8. Ability to communicate via phone in noisy and hectic environment.
9. Demonstrate the ability to accomplish time sensitive task with a reasonable time frame.
10.Ability to work anytime of the day, independently or with minimal supervision. Be detail oriented, alert and self-motivated.
11.Ability to effectively work with and manage difficult situations, staff and clients and their family.
12.Be willing to drive through areas and go into homes that are located in high risk and high crime areas.
13.Exposure to animals, pests, communicable diseases and hazardous waste is possible.
