Licensed Practical Nurse / Licensed Vocational Nurse (Remote)

KangarooHealth is looking for a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) licensed in California to join our remote clinical team. This role is ideal for someone who puts patients first, communicates with compassion, and thrives under pressure. As a KangarooHealth nurse, you’ll support patients with chronic conditions like Hypertension, Diabetes, Heart Failure, and COPD through our digital platform—helping them stay connected, monitored, and cared for between visits.
What You’ll Do
You’ll conduct daily outreach to patients, review their vitals, provide health coaching, and support their use of connected devices. You’ll troubleshoot issues, escalate concerns to providers when needed, and help patients feel supported and seen every step of the way.

Requirements:
Active LPN or LVN license with multi-state compact privileges as LPN
Technical fluency with ability to work in multiple platforms and systems ie G-Suite, Microsoft Office, Hubspot, Slack, Zoom, Epic, ECW, Athena, etc)
5+ years clinical experience (hospital, family, or geriatric care)
5+ years EMR experience and ability to learn new platforms quickly
1+ year experience working remotely and engaging patients by phone
Strong understanding of chronic conditions and care management
Proficiency in documenting RPM/CCM codes (e.g., 99453, 99454, 99457, 99458)
Bilingual fluency in Russian or Spanish preferred
Comfortable using video conferencing and digital workflows
Reliable internet, computer, and a private, HIPAA-compliant workspace
Must be available to work 40 hours per week aligned with client clinic hours (e.g., 9AM–6PM local time)
Key Functions:
Outreach and follow-up for remote patient monitoring and vitals tracking
Medication reconciliation and lifestyle coaching based on care plans
Documentation and escalation to providers as needed
Assist patients with device setup and troubleshooting
Maintain compliance with HIPAA and company protocols
Support onboarding and training of new team members
Compensation & Benefits:
$55,000-$65,000 total compensation
Base pay plus performance-based bonuses
