ABOUT THE ROLE

The Respiratory Therapy Care Coordinator provides remote clinical support, care coordination, and structured follow-up for patients receiving respiratory services and equipment in the home setting. This role focuses on virtual patient engagement, plan-of-care monitoring, troubleshooting respiratory equipment issues, and supporting adherence to therapy over a defined 90-day care period.

This position is designed to enhance patient outcomes through proactive virtual touchpoints, early identification of clinical or equipment concerns, and coordination with field Respiratory Therapists, physicians, and internal clinical teams. This is a remote role and does not include in-home patient visits.

WHAT YOU’ll DO

  • Provide virtual respiratory care coordination and follow-up for patients
  • Conduct scheduled and as-needed telephonic or virtual check-ins to assess patient status, adherence, and equipment tolerance.
  • Troubleshoot respiratory equipment issues remotely, including oxygen, ventilators, PAP devices, and tracheostomy-related supplies (within scope of licensure and escalation protocols).
  • Support patient and caregiver education on equipment use, safety, therapy adherence, and troubleshooting best practices.
  • Monitor patient progress and identify changes in condition, escalating clinical concerns to field RTs as appropriate.
  • Coordinate with field Respiratory Therapists to ensure continuity of care and timely in-home intervention when needed.
  • Maintain accurate, timely documentation of all patient interactions in the electronic health record in accordance with company standards.
  • Support onboarding of new patients into respiratory services, ensuring they understand therapy plans and equipment use prior to and after setup.
  • Reinforce plans of care and help patients remain compliant and engaged throughout the 90-day care cycle.

WHAT YOU'LL BRING

  • Current Respiratory Therapist license (RRT or CRT) required; must maintain licensure in all applicable states of practice.
  • BLS certification required.
  • Experience in home respiratory care, durable medical equipment (DME), acute care, pulmonary rehabilitation, or related settings preferred.
  • Strong clinical understanding of oxygen therapy, ventilators, PAP therapy, and tracheostomy care.
  • Ability to assess patient needs and recognize when escalation or in-person intervention is required.
  • Comfortable working independently in a fully remote environment while collaborating closely with field clinical teams.
  • Strong communication, patient education, documentation, and problem-solving skills.
