\u2022 Travel across the metro to support different territories and care teams<\/b>.
<\/div>
\u2022 Provide skilled nursing visits, assessments, and care planning<\/b>.
<\/div>
\u2022 Complete OASIS visits and coordinate patient care<\/b>.
<\/div>
\u2022 Build trust quickly with patients and families.
<\/div>
\u2022 Help maintain consistent care during growth or staffing changes<\/b>.
<\/div>

What We\u2019re Looking For<\/b>
<\/div>
\u2022 Active Minnesota Registered Nurse (RN) license.
<\/div>
\u2022 Comfortable working independently in a mobile, field\-based role<\/b>.
<\/div>
\u2022 Strong clinical judgment and adaptability.
<\/div>
\u2022 Home health experience preferred, but not required.
<\/div>
\u2022 Reliable transportation and willingness to travel.
<\/div>
