What You\u2019ll Do<\/b>

skilled nursing visits<\/b> in homes and community settings.
<\/div>\u2022 Perform
assessments, wound care, and medication management<\/b>.
<\/div>\u2022 Educate patients and families on care plans and recovery.
<\/div>
\u2022 Adapt quickly to new territories and patient needs.
<\/div>
\u2022 Collaborate with RN Case Managers and interdisciplinary teams.
<\/div>
\u2022 Maintain
accurate, timely documentation<\/b>.
