Senior Community Health Worker - Quality Revenue Enhancement
The Senior Community Health Worker is responsible for helping community residents and Providence Medical Foundation patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The Sr. Community Health Worker will support in the following areas:
a) Health Education: Assist with recruitment and administrative tasks for health education courses, such as but not limited to Mental Health First Aid, Creating Healthier Attitudes Today, physical activity, or Groceryships. In addition they will deliver these modules these health education topics independently and identify new locations where the programs can be delivered.

Required Qualifications:
- Education to meet certification, license or registration requirement.
- Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
- 2 years of Experience in a combination Community Health Worker I and II position, or 2 years related community outreach, enrollment, education or other community health administrative experience, or Bachelors degree in lieu of experience.
