Senior/Medical Social Worker (Community Settings/Multiple Locations Available/5 days)

• Conduct comprehensive biopsychosocial assessments for patients, residents, clients, and caregivers, and implement appropriate interventions in line with established social work professional standards.
• Manage the full spectrum of social work services, including assessment, care coordination, intervention planning, counselling, emotional support, monitoring, and outcome evaluation.
• Address clients’ psychosocial and financial concerns, while contributing to discharge and transition planning as part of integrated care management.
• Work collaboratively with healthcare professionals such as physicians, nurses, therapists, and other allied health staff to deliver coordinated and person-centred care.
• Engage and reach out to individuals and communities who may benefit from group work interventions and support programmes.
• Provide mentorship, training, and shared supervision for social work students on field placements and newly appointed Medical Social Workers, fostering professional growth and adherence to best practices.

Requirements:

• Degree in Social Work (or major in Social Work) from a recognised institution
• 2–3 years of relevant experience in social work, preferably in healthcare or community settings
• Experience in case management,including psychosocial assessment, intervention, and care planning
