Job Title: Settlement Health Coordinator

Location: St. John’s, NL

Position Type: 1 full-time position until March 31, 2027, with possibility of extension pending budget availability and satisfactory performance

Reporting to the Social Work & Health Services Manager, the Settlement Health Coordinator is responsible for the day-to-day supervision of the Settlement Health team and coordinating health-related services to clients. This role will ensure that clients have access to health services and appropriate support. The Settlement Health Coordinator will work closely with health service providers and lead the internal team for a seamless client experience.

Roles and Responsibilities:

  • Connect newcomers with health services, including primary care, mental health services, and specialized healthcare
  • Liaise with healthcare providers and community organizations to ensure clients have access to culturally appropriate healthcare
  • Conduct assessments to identify health needs of clients, including barriers to accessing healthcare services
  • Provide individualized support and case management to clients requiring additional assistance with navigating the healthcare system
  • Assist clients with completing health-related forms and documentation Build partnerships with local health authorities, community health centers, and social services agencies Collaborate with internal teams to provide holistic support to clients
  • Advocate on behalf of clients to ensure they receive appropriate healthcare and support services
  • Track client progress and maintain accurate records in client management systems
  • Monitor team performance
  • Writing reports and meeting compliance requirements of funders

Qualifications and Experience:

  • Bachelor's degree in Public Health, Social Work, Community Development, or related field (or equivalent experience)
  • Minimum of 2-3 years of experience in community health, social services, or settlement services, preferably working with immigrants and refugees
  • Experience in program coordination, case management, and client support
  • Strong knowledge of local healthcare systems and services
  • Excellent communication and interpersonal skills, particularly in working with diverse populations
  • Ability to speak a second language will be considered an asset
  • Strong organizational skills and ability to manage multiple tasks and deadlines
  • Familiarity with trauma-informed care and mental health services for vulnerable populations

Conditions of Employment:

  • Legal authorization to work in Canada
  • Flexible to work outside of core hours (Monday-Friday, 9:00am - 5:00pm)
  • Valid class 5 NL driver’s license and daily access to a personal vehicle is required
  • Ability to travel inside and outside the province is required
  • Position is subject to a satisfactory Criminal Record Screening and Vulnerable Sector Check from the Royal Newfoundland Constabulary
