Social Worker (Age Well Department - South West, Community Case Management Service)

You will be part of Fei Yue’s Community Case Management Service (CCMS) team, providing support to seniors with complex care needs and their caregivers while managing the multiple care needs through home visits.

Responsibilities:

Conduct comprehensive assessments of clients’ psychosocial, medical, emotional, and functional needs to determine appropriate interventions.
Develop, implement and review individualised care plans based on clients’ needs and goals.
Provide case management support through regular follow-ups, monitoring and intervention.
Coordinate and liaise with healthcare providers, community partners, government agencies, and other stakeholders to facilitate service delivery.
Advocate for clients’ needs and support access to appropriate resources and services.
Maintain accurate and timely case documentations, reports, and records in accordance with organisational standards.
Participate in multidisciplinary case discussions, case conferences and team meetings.

Qualifications

Degree in Social Work
At least 2-3 years of relevant working experience in case management in eldercare or healthcare setting preferred.
Good knowledge of Singapore’s social service and healthcare landscape.

Requirements

Strong critical thinking and problem-solving skills
Proficiency in written and spoken English and Mandarin. Additional languages or dialects would be advantage in engaging with seniors.
Strong interpersonal and communication ability
Resilient and adaptable in challenging situations.
Strong team player with excellent organizing and administrative skills

Office location: Teck Whye

Home visit location: Mostly northwest of Singapore, occasionally northeast of Singapore
