The person in this position performs a variety of duties to address the social, psychological and economic needs of children and their families, in relation to acute and chronic illness.
The Social Worker PRN is concerned with continuity of care which usually involves discharge planning for children who are inpatients and their families to ensure they understand and follow the recommended treatment plan, and referrals to community agencies and programs. All duties are performed in accordance with established policies and procedures and within the professional standards of the social work profession as defined by the National Association of Social Worker's Code of Ethics.
- Location: Milwaukee Hospital
- Areas: EDTC, Inpatient Units and the Craig Yabuki Mental Health Walk-In Clinic
- Hours: Evenings, weekends and holidays
- On Call: Overnights with a 30 minute or less response time
Experience:
1+ years of previous social work or related field experience required
Hospital experience preferred
Education:
Master's Degree MSW required
Licenses and Certifications:
One of the following:
License Social Worker Advance Practice - APSW - (121) - State of Wisconsin
License Social Worker Clinical (123) - State of Wisconsin
License Social Worker Independent (122) - State of Wisconsin
Essential Functions:
Understands and satisfies the needs of the neonatal, infant, toddler, pre-school, school-age, adolescent and/or adult patient in regards to their growth and development process.
Assists families in meeting their needs through: Short term, crisis oriented counseling; referrals to community agencies, programs and services; educating patients/families about illness, hospital procedures and community resources.
Helps plan for the future health care needs of the child whether or not admitted to the hospital.
Consults with other professional staff to assure that they understand the strengths and needs of patients and families. Assesses the medical care/history of the child and family to assist medical staff with diagnosis and treatment plan.
Performs duties in courteous and helpful manner with respect towards patients/families, department staff, visitors and employees from other departments.
- Requires flexibility to be available for call, as assigned, to meet department needs in the care for patients/families.
Please Note: This role does not provide supervision for clinical hours toward licensure.
