Social Worker - PRN

Providence Care is seeking a compassionate and experienced PRN Social Worker to support hospice patients and their families.

In this role, you will provide emotional support, resource coordination, and guidance through end-of-life care; working closely with an interdisciplinary team to ensure patients receive care with dignity and respect.

What You Will Do

• Complete psychosocial assessments for patients and families
• Provide counseling and emotional support
• Assist with care planning alongside the clinical team
• Connect families with community resources and services
• Support bereavement needs as appropriate
• Educate patients and families on available services and options
• Maintain accurate and timely documentation
