St. Hilda's Community Services Centre is a voluntary welfare organisation in Singapore serving Tanjung Rhu and nearby areas. It operates a Day Care Centre, Day Rehabilitative Centre, and an Active Ageing Centre to support community needs.

Job Summary

You will conduct casework, counselling, and home visits to support residents. Advise on social assistance schemes, assess referrals, develop care plans, and collaborate with agencies to deliver community programs and manage volunteers.

Care Management:

  • Provide case management for clients to support them in the community.
  • Befriend and outreach to clients/families/caregivers/residents in the community.
  • Assist in identifying clients with early onset of diseases or mental health conditions e.g. dementia, depression, anxiety etc
  • Assess, review, coordinate and participate in multiple health, social care support to clients who require program services, that includes putting up referrals, social intake, medical escort, etc.
  • Participate in multi-disciplinary case / patient discussion on regular basis and provide financial and social care inputs.
  • Work with other clinical professionals or relevant agency in looking into comprehensive care including advocacy work for clients.
  • Assist in means testing and financial grants application to meet medical/social care needs.
  • Source for financial assistance schemes / subsidies/ charity funds for clients with financial difficulties.
  • Provide counselling/support to clients and their families who require help.
  • Organise support group and training e.g. group counselling and awareness talks to caregivers / families / volunteers/staff.

Service Quality:

  • Ensure good & measurable outcomes & quality of care; good client’s satisfaction; timeliness and proper documentation of client records.
  • Ability to initiate / support Quality Improvement projects or programs in the aspects of psychosocial overall care.
  • Ability to work in multi-disciplinary team to deliver medical and psychosocial care to clients.
  • Case work management skills & presentation skills.

Training & Development:

  • Professional development and training courses will be provided for enhancement of skills and competency.
  • Social Workers / Counsellors will receive training in medical, mental health, palliative care, vulnerable person protection, counselling and family/community social work.
  • On-the-job training will be provided for fresh graduates.
