Job Description:
The Inpatient Palliative Care Unit at Trillium Health Partners (THP), located at the Mississauga Hospital (MH) site, consists of 16 beds dedicated to providing specialized palliative and end-of-life care to patients with life-limiting illnesses.
Working in close collaboration with palliative care physicians and an interprofessional team, the Speech Language Pathologist is a highly skilled, adaptable clinician with strong critical thinking and communication abilities. In this role, you will provide patient- and family-centred care focused on optimizing comfort, function, and quality of life. Care delivery emphasizes symptom management, maintenance of mobility and dignity, and support for patients and families navigating complex and sensitive stages of illness.
Recognizing the unique needs of this population, care is guided by patient preferences, functional tolerance, and evolving goals of care. Family involvement and education are integral components of care delivery.
Reporting to the Clinical Manager, the ideal candidate is committed to delivering exceptional, compassionate care in a dynamic and emotionally complex environment. Qualifications:
- Master’s degree in Speech-Language Pathology from an accredited institution.
- Licensed in good standing with the College of Audiologists & Speech-Language Pathologists of Ontario (CASLPO)
- Previous experience with assessment and management of communication and swallowing disorders in a hospital setting, including video- fluoroscopic assessment
- MBSImp certification is preferred or willingness to be certified
- Candidates with recent clinical experience, knowledge, education, or certification in palliative care preferred
- Basic Cardiac Life Support certification
- Demonstrated commitment to ongoing professional development and student learning
- Strong case management skills
- Superior interpersonal communication skills, including verbal and written communication
- Interest in potential professional and career development opportunities
- Satisfactory attendance and performance required
- Experience with complex geriatric patients with co-morbid neurological, surgical and medical conditions
- Experience in providing education to patients and families in individual and group settings
- Experience with end of life care is preferred
- Experience with tracheostomies is preferred
Responsibilities:
- Assessment of patients with communicative, swallowing and cognitive-communication dysfunction of the cardio-pulmonary, musculo-skeletal and neurological systems and co-creation of goals with the patient and family
- Planning and implementation of treatment plans to improve communication, swallowing ability and overall participation in life activities
- Participate in patient and family education on feeding and treatment plans
- Participate in patient discharge planning
- Document patient assessment and treatment in the health record
- Collaboration with the inter-professional healthcare team, patients and their families to achieve patient goals and augment the treatment plan as needed
- Providing education to the inter-professional team to support evidence-based care
- Assigning to and supervision of support personnel
- Involvement in clinical education of various professions, including preceptorship of SLP student
- Participate in weekly interprofessional progress rounds, monthly professional practice meetings, business meetings and program meetings
