Staff Nurse (Age Well - North East, HC)

Responsibilities:

Comprehensive Nursing Assessment & Care Planning

  • Conduct holistic nursing assessments for clients in the home setting
  • Develop, implement, and review individualised care plans based on clients’ medical, functional, psychosocial, and caregiving needs
  • Monitor clients’ progress and evaluate effectiveness of care interventions
  • Review and update care plans regularly or whenever clients’ conditions change

Clinical Nursing Care & Interventions:

Provide skilled nursing care and clinical procedures, including:

  • Wound assessment and wound management
  • Enteral feeding tube management and replacement
  • Urinary catheter care and management
  • Medication administration and management
  • Chronic disease monitoring and nursing interventions
  • Manage acute and chronic medical conditions in consultation with doctors and multidisciplinary healthcare teams
  • Identify changes in clients’ conditions and initiate timely medical interventions where necessary

Caregiver Training & Health Education

  • Educate clients and caregivers on disease management, medication, and preventive care
  • Equip caregivers with practical caregiving skills and confidence through hands-on training and guidance
  • Support clients and families through emotional, psychological, and spiritual care

Care Coordination & Case Management

  • Coordinate follow-up visits and maintain regular communication with clients and caregivers throughout the care journey
  • Maintain accurate and timely documentation of nursing assessments, care plans, and case records
  • Work collaboratively with healthcare professionals and community partners to ensure holistic and integrated care support

Teamwork

  • Contribute actively within a multidisciplinary team to deliver safe, quality, and person-centred home care services

Requirements:

  • Minimum a Diploma in Nursing
  • Registered Nurse with valid SNB practising certificate
  • Relevant nursing experience in community care, home care, geriatrics, or chronic disease management will be an advantage
  • Possess good communication, interpersonal, and care coordination skills
  • Comfortable working independently in clients’ homes and community settings while collaborating effectively within a team.
  • Candidates with experience in wound care, enteral feeding, catheter care, and caregiver training are preferred
  • Strong clinical competency and nursing judgement
  • Compassionate, patient-centred, and service-oriented mindset
