Job Description:

Responsibilities:

• To perform home visits to palliative care patients.
• Performing nursing assessment on the patients.
• Initiating nursing care plan and implementing new measures.
• To educate caregivers/families on basic nursing procedures.
• To refer patients to other professional bodies such as inpatient hospice when the need arises.
• Preparing families for approaching deaths.
• Providing family with emotional and bereavement support

Requirements:

• Fully registered with the Singapore Nursing Board(SNB).
• Meticulous with strong attention to detail.
• Minimum of 3 years of relevant working experience.
• Possess good interpersonal and communication skills.
• Able to work independently with minimal supervision.
