Team Leader, Flexible Housing Pool

Thresholds is seeking a compassionate and highly organized Team Leader to support individuals served through our Flexible Housing Pool (FHP) program—people navigating housing instability alongside complex health and social needs.

The FHP program serves individuals experiencing or at risk of homelessness, many of whom face overlapping challenges such as chronic medical conditions, behavioral health needs, substance use, or limited access to income and community supports. While not all members have a diagnosed mental illness, many require thoughtful coordination across multiple systems to remain stable. At its core, this role is about helping people obtain and maintain safe, stable housing—and using that housing as a foundation for improved health and long-term stability.

To Succeed in this role, you need to:

  • Onboard, train, collaborate with and supervise a team of Service Coordinators
  • Support the mental and physical health care of members on an assigned caseload.
  • Closely coordinate care with member’s mental health, physical health, and/or substance use providers.
  • Work closely with providers, community resources and organizations to coordinate member services and treatment needs.
  • Provide support and attempts to remove common obstacles and barriers to outpatient care.
  • Identify risk factors related to relapse and develops relapse prevention plans and strategies.
  • Advocate for members who may be vulnerable or need additional support.
  • Provide member education on common mental health, physical health, and substance use disorders.
  • Provide brief interventions using evidence based psychosocial treatments, (CBT, MI) as clinically indicated.
  • Participate in regularly scheduled caseload consultation emphasizing focus on members who are new to treatment, difficult to engage, or who are not improving as expected.
  • Facilitate member outreach, engagement, linkage, referrals, coordination, and follow up for mental health, physical health, and substance use services and supports.
  • Identifie and addresses gaps in care for members and arranges appropriate and timely resolution with internal and external agencies.
  • Work cooperatively with inpatient and emergency hospital staff. Performs and participates in crisis intervention when necessary.
  • Ensure grant compliance with program targets by identifying and linking participants who are in need of income, employment, or are eligible for benefits and assists in connection to employment services and coordinates mainstream benefits applications.
  • Ensure compliance with service delivery, data quality, and program outcomes for permanent supportive housing programs.
  • Collaborate closely with Thresholds Housing Administration, HMIS data administrator and Grants Accounting Team through the submission of partner service reports.
  • Monitor service delivery of project service partners to program participants, including reviewing personnel activity reports.
  • Provide accurate and timely information to their supervisor(s) on all topics and issues related to the project operations and wellbeing.
  • Meet required outreach targets and completes all documentation within assigned time frames.
  • Other duties as assigned.

Many education and experience paths are eligible for this role:

  • High school diploma or GED certificate and 5 years of supervised clinical experience
  • Bachelor’s degree in Psychology, Social Work, or related field
  • Bachelor’s degree in an unrelated field and 2 years of supervised clinical experience
  • Master’s degree in Psychology, Social Work, or related field
  • Previous supervisory experience preferred
