Temp Assistant Care Coordinator (Healthcare) #HZOT

Location: Multiple locations / West
Duration: 1 year renewable
Hours: 5.5 days, office hours

Job scope:

• Build strong relationships with patients, caregivers, and healthcare professionals
• Coordinate and support patient care activities using healthcare IT systems
• Track, document, and monitor patient care interventions and clinical indicators
• Perform basic health parameter measurements (e.g. blood pressure, weight, glucose monitoring)
• Support national preventive health screening and vaccination initiatives
• Provide basic health education on lifestyle management, physical activity, and healthy living
• Promote social prescribing and increase awareness of community resources available to patients
• Educate patients on the use of health monitoring devices and vaccination requirements
• Support efforts to reduce hospital readmissions and improve patient outcomes through preventive car

Requirements:
- Min. O / A-level / Nitec / Diploma in any field
- Some customer service experience required!
