Scope of Services -Tuia Ngā Taura Kaiāwhina

The service supports whānau whose tamariki have been discharged from Waikato Hospital, or who have been referred from a Waikato outpatient clinic or Kāinga Ora and includes the following:

  • Working alongside whānau to complete a whānau hauora assessment (WHA), conducted either during home visits or via phone to whānau.
  • Providing health and social service education, with referral to additional services as required
  • Supporting to health and social services as required including advocating for whānau at appointments if appropriate
  • Participation in multidisciplinary team hui and collaborative care planning
  • Working as part of a nurse–kaiāwhina team, escalating any clinical concerns to the nurse or GP
  • Speaking and presenting to external services as part of the team to promote the benefits of the Tuia Ngā Taura service and to encourage appropriate referrals

Clinical and Cultural Standards

The contract is to be delivered in alignment with high clinical and cultural standards. The role is required to:

  • Actively participate in Continuous Quality Improvement (CQI) reviews and related activities.
  • Reflect and integrate kaupapa Māori values across all aspects of service delivery.
  • Ensure seamless transitions of care, strengthen whānau capability, and contribute to improved health outcomes for tamariki through culturally grounded, whānau-centred practice
