Key Responsibilities
Enable Seamless Care Operations
- Support Ops Team Leads and work closely with clinicians, nurses and allied health professionals to ensure smooth day-to-day programme operations
- Coordinate patient journeys from enrolment to discharge, ensuring efficiency and accuracy at every touchpoint
- Manage scheduling, appointments and care transitions to support continuity of care
Deliver an Excellent Patient Experience
- Conduct service introductions and facilitate patient onboarding and consent for WH@Home enrolment
- Provide financial counselling support and assist with subsidy applications and documentation
- Maintain clear, accurate and timely documentation of patient interactions in systems such as EPIC
- Handle patient feedback, enquiries and service recovery with professionalism
Support Clinical Operations
- Prepare patient case files for home visits and teleconsultations
- Assist with patient discharge processes and required documentation
- Adhere to infection prevention protocols when handling patients and equipment
Manage Medical Equipment & Supplies
- Prepare, assemble and maintain self-monitoring kits and medical supply kits for patients
- Conduct quality checks, track inventory and ensure proper documentation
- Coordinate delivery and collection of equipment, ensuring strict infection control and disinfection standards
Strengthen Programme Coordination
- Arrange patient transport and concierge services to support care delivery
- Coordinate ambulance services where required
- Support onboarding/offboarding of multidisciplinary team members
- Manage departmental communications and follow-ups
Drive Administrative Excellence
- Support scheduling, meetings, training and programme-related activities
- Maintain rosters and ensure operational readiness beyond office hours
- Provide comprehensive administrative support and contribute to process improvements
Work Schedule
- 5-day work week with standard working hours from 830am to 6pm on weekdays, including weekday and weekend shifts
- Staff may beactivated on an ad hoc basis during periods of hospital surge, including public holidays and weekends, with working hours to be determined based on theoperational situation at the time (either half day or full day up to 6:00pm). Staff activated during these periods will be compensated with time off
What You Bring
Experience & Skills
- Diploma, GCE ‘A’ Level or equivalent, with 1–2 years of relevant experience preferred
- Experience in healthcare or customer-facing roles is advantageous
- Proficient in Microsoft Office (Excel, Word, PowerPoint)
- Comfortable working with data entry, documentation and coordination tasks
Attributes
- Strong communication skills in English; proficiency in Mandarin, Bahasa Melayu or dialects is an advantage
- Detail-oriented with the ability to manage multiple tasks and timelines
- Positive, proactive and service-oriented mindset
- Strong interpersonal and coordination skills with a patient-centric approach
- Able to thrive in a fast-paced, dynamic environment both independently and as part of a team
