Care Coordinator

Responsibilities

• Coordinate patient care and follow-up appointments.
• Educate patients on preventive health, screenings, and chronic disease management.
• Monitor patients' health status and identify care needs.
• Liaise with doctors, nurses, caregivers, and community partners.
• Assist patients in using digital health tools and home monitoring devices.
• Maintain accurate patient records and documentation.

Requirements:

• Diploma in a healthcare-related field or relevant work experience.
• Strong communication and interpersonal skills.
