Nurse (Clinical/Case Management)

About the Position: This position is located at Army Health Clinic Munson, Fort Leavenworth, Kansas. This is a Direct Hire Solicitation A recruitment incentive may be authorized. Non-Federal work experience may be creditable for leave purposes. Salary negotiation may be available for those candidates who are new to Federal service.

Coordinate healthcare services, referrals, and resources across military and civilian providers to ensure timely patient care.

Analyze complex medical, psychosocial, and administrative problems to recommend effective solutions and improve outcomes.

Provide comprehensive assessments of patient health needs and develop individualized, cost-effective care plans.

Evaluate patient progress through follow-up assessments, documenting interventions and guiding informed decision-making.

Collaborate with multidisciplinary teams and serve as patient advocate to minimize barriers and optimize quality of life.
